ACCEPTANCE FACILITY

PROCESSING TIMES

 

NEW PASSPORT BOOK OR CARD (Ages 16 and up)

You will need:

  • One color passport photograph, 2"x2" with a white background (taken no more than 6 months ago)
  • Proof of Citizenship - Certified Birth Certificate (must show applicant's full name, date & place of birth, raised/embossed seal and file date); Expired US Passport; Certificate of Naturalization
  • Proof of Identity - You must present your original identification AND submit a photocopy of the front and back with your passport application
  • Completed DS-11 form
  • Check or Money Order made out to the US Dept of State (Refer to Table)
  • Cash, Check or Money Order made payable to the City of Salem for $35.00

 

NEW PASSPORT BOOK OR CARD (Ages 15 and under)

You will need:

  • One color passport photograph, 2"x2" with a white background (taken no more than 6 months ago)
  • Proof of Citizenship - Certified Birth Certificate showing the applicant's full name, date & place of birth, raised/embossed seal and file date. Under age 16, parents' names must also appear on the certificate; Certificate of Naturalization
  • Evidence of the child's relationship to parents/guardian(s)
  • Original parental/guardian government-issued identification AND a photocopy of the front and back side of presented identification.
  • Both parents and child must be present when submitting application. If only one parent appears, you must also submit one of the following:
    • Second parent's notarized written statement consenting to passport issuance for the child with photocopy of second parent's ID
    • Second parent's death certificate if second parent is deceased
    • Primary evidence of sole authority to apply
    • A written statement (made under penalty of perjury) explaining in detail the second parent's unavailability
  • Completed DS-11 form
  • Check or Money Order made out to the US Dept of State (Refer to Table)
  • Cash, Check or Money Order made payable to the City of Salem for $35.00

Passport Fees

 

PASSPORT RENEWAL (completed and mailed by yourself)

You may use form DS-82 if:

  • You can submit your most recent US Passport
  • You were at least 16 years old when your most recent US Passport was issued
  • You were issued your most recent US Passport less then 15 years ago
  • You use the same name as on your most recent US Passport  OR  You have changed your name by marriage or court order and can submit proper documentation to reflect your name change

** If you are unable to meet all of the above specifications, you are considered a new applicant **

To renew your passport, you will need:

  • Your most recent US Passport
  • One color passport photograph, 2"x2" with a white background (taken no more than 6 months ago)
  • A marriage certificate or court order if your name has been changed
  • Check or Money Order made out to the US Dept of State

Forms are available in our office, or by visiting the US Department of State's website at travel.state.gov.

Customers who are traveling within 2 weeks (14 days) or who need foreign visas for travel must make an appointment at the National Passport Information Center at 1-877-487-2778, Monday-Friday 8:30am - 4:30pm local time, excluding Federal holidays.
