PLEASE BE ADVISED THAT YOU CANNOT FILE A POLICE REPORT ON THIS FORM.  YOU MUST CALL 978 744 1212 TO FILE A POLICE REPORT. 

THIS FORM IS USED FOR COMPLAINTS AGAINST, OR COMMENDATIONS OF, A POLICE OFFICER OR OTHER POLICE DEPARTMENT STAFF.

Upon filing this complaint/commendation, you should be taken to a page that says "Receipt of your message is acknowledged".  If you are not taken to the acknowledgement page, then assume non-receipt and notify the police department at (978) 744-0171 extension 50110 or or by email at: admin@salempd.net

By filing a complaint, I understand that I will be informed of the result of the investigation and the disposition of my complaint within thirty (30) days, unless otherwise advised. I am willing to testify at any Internal or Criminal Hearing on this matter. I agree that to the best of my knowledge, the below statement is a true and accurate account. 

All commendations will be reviewed by The Chief.
